In Wiesendangen, he began his youth career with the local club. He  e joined FC Winterthur as a member of the youth team in 2007. He was a member of the Under-18 team and then the second team. Akanji prefers to play defensively at center-back, but because he can play with either foot, he has also been successful on the right and left. He was in Winterthur’s starting eleven in the 2014–15 Challenge League after appearing in two games for them the previous season. On April 15, 2015, it was reported that Akanji would join Basel for the 2015–16 Super League season. On September 26, 2015, he made his Swiss Super League debut as a substitute against FC Lugano.

Akanji won the Swiss Super League championship for the first time at the end of the 2015–16 season, and for the second time at the end of the 2016–17 season. Liverpool were tracking Basel’s teenage defender Manuel Akanji in December 2017, but Borussia Dortmund were also interested. Dortmund are reportedly interested in signing Akanji in the January transfer window, while Liverpool are more open to a summer deal. Basel announced Akanji’s transfer to Borussia Dortmund on January 15, 2018. This was for a reported cost of 18 million euros during the winter transfer window.
